After an Audit is submitted, Opus gives you tools to track results, monitor completion, and resolve any flagged issues through a structured remediation workflow.

Audit Remediation

When steps in an Audit require remediation, Opus automatically starts a remediation workflow to make sure every issue gets resolved, reviewed, and approved before it's closed out. Learn how to add remediation steps to your Audit here.

Step 1: Assign the Remediation Form (Auditor)

After a user submits an Audit with steps that require additional attention, Opus prompts them to assign a remediation form.

  • The form includes all steps that need attention.

  • The auditor selects an assignee. Only users at the submitted Audit's location can be selected.

  • The auditor can set a due date and add any relevant notes before sending.

Step 2: Complete the Remediation Form (Asignee)

Once assigned, the remediation form appears on the assignee's home screen in the Opus Training app. They are also notified via push notification.

  • The assignee fills out the form, logging what they did to resolve each flagged step.

  • Once all steps are addressed, the form is submitted back to the auditor for review.

Step 3: Review and Approve (Auditor)

After the assignee submits the form, it returns to the auditor in the Opus Training app.

  • The auditor reviews each step and can approve it if the issue is resolved, or reject it if more work is needed.

  • If a step is rejected, a new remediation form is created and the cycle starts over.

  • This continues until the auditor approves every flagged step.

Audit Reporting

Audit reporting allows users to review submissions, monitor completion, and identify issues across locations.

Reporting in the Dashboard

To access reporting, open an Audit in the dashboard and select the "Reporting" tab. From there, you can switch between views depending on what you want to analyze.

  • All Submissions View a complete list of every Audit submission, including who submitted it, when it was completed, the overall score, and any flagged issues. Click into any submission to review the full details and all responses.

    • In the grid view, you can export submission data as a spreadsheet.

    • Enable "Show Full Submission Detail" to include every step in the Audit along with its response.

  • Schedule Completion Track how consistently Audits are being completed within their scheduled windows. This view helps you quickly identify locations that submitted on time, submitted late, or missed their scheduled window.

You can also view Audits in the Audit Completion Grid. This helps you compare completion status across multiple Audits and locations side by side, making it easier to spot gaps in compliance or participation.

The Audit Completion Grid is a pre-built reporting view. You can find it in the Reporting section of the dashboard.

Reporting in the Opus Training App

Users with access to an Audit can review past submissions directly in the Opus Training app.

  1. Open the Audit from your Library in the app to review past submissions.

  2. Click into any submission and access the "Activity" section

  3. Select "View submission" to see the full details, including all questions and responses.

  4. Select "View form" to see the full remediation form, including what has been taken action on.

Where do I find Audit reports?

Open the Audit in the dashboard and go to the "Reporting" tab. Alternatively, you can use the Audit Completion Grid, which is a pre-build report in the Reporting section of the dashboard.

Where can I see Audit scores and results?

Any user with access to the Audit can see the results in the Opus Training app and the Opus dashboard.

Can I export Audit data?

Yes. In the reporting tab of an Audit in the dashboard, you can export submissions as a CSV or save individual submissions as PDFs.

What does the Schedule Completion view show?

It shows how well each location is completing Audits within scheduled windows. Creating schedules for Audits is optional but can be helpful to remind your team of when an Audit needs to be completed.

What happens if a remediation step gets rejected?

The auditor rejects the step, which creates a new remediation form. The assignee completes it again and the auditor reviews again. This repeats until the step is approved.

Who can be assigned a remediation form?

Only users at the same location where the Audit was submitted can be selected as the assignee.

Where do assignees find their remediation forms?

Remediation forms appear on the assignee's home screen in the Opus Training app.
